Key documents for service delivery
Why is it important?
Cardiovascular Disease (CVD) is one of the main causes of death and disability in England and is linked to modifiable risk factors, such as smoking, physical inactivity, poor diet, high blood pressure and high cholesterol.
Non-communicable disease and disability has the greatest impact on the most deprived communities, so the NHS Health Check provides an opportunity to address entrenched health inequalities.
The NHS Health Check also provides an opportunity to identify and refer at-risk people into services such as LiveWell Dorset, who help patients address lifestyle factors to reduce their risk of developing CVD.
